This portal is not intended for transfer of complete medical records from previous healthcare providers.
Please note that you will need the Health Card number and date of birth of the person for whom the photo and/or document is being submitted. For any forms, please ensure that the patient section has been completed and signed. Omission of a signature may delay the completion of the form.
Form Completion Timeline
Due to the high volume of forms and documentation requests received by our clinic, patients are asked to allow up to six (6) weeks for completion of all forms. Our providers must balance these requests with their ongoing patient care responsibilities.
The size limit per attachment is 10MB and the accepted file types are .jpg, .jpeg, .png, .tiff, .tif, and .pdf.
Sensitive photos (including breasts, genital area, and buttocks) should not be sent.
Appointments cannot be requested through this portal. To book an appointment please call the office to speak with reception or go to the Online Booking page.
The portal is not monitored outside of business hours. Do not use this portal for any message requiring a reply.
