REGISTERED PRACTICAL NURSE- 12-Month Contract Float Position – Travel to all sites is required

Job description:

The Registered Practical Nurse is a member of the interprofessional team at WRNPLC and provides clinical care and clinical administrative functions to support the delivery of high-quality patient centered primary care in the NPLC setting. The individual also participates in the development, implementation, monitoring and evaluation of programs and services for clients, their supports, and the community. This role is a float position; travel to the Kitchener, Breslau and Cambridge sites is a requirement for this role. **MUST have reliable transportation and a valid driver’s license to provide service at all sites**

 Pay: CA$25.89 – CA$28.08 per hour

REQUIREMENTS:

  • Graduate of a registered practical nursing program from a recognized institution
  • Certificate of Competence from the College of Nurses of Ontario
  • Three to five years nursing experience, preferably in a community setting
  • Trained and skilled in Spirometry and Foot Care is an asset
  • Experience in program development, implementation, monitoring and evaluation
  • Proficiency in the use of computers, electronic medical records (Practice Solutions experience an asset) and various software application
  • Excellent interpersonal skills, including written communication and skill in challenging interactions.
  • Proven expertise in both teamwork and independent functions
  • Welcoming of change and innovative in change management
  • Ability to maintain information in confidence.
  • Open and nonjudgmental
  • Desire and ability to update knowledge and skills.
  • Demonstrated capacity to deal effectively with multiple and complex work demands accurately, effectively, and efficiently
  • Second language an asset

Primary Care Nurse Practitioner- Addictions Subspecialty

(Full time, permanent- 37.5 hours per week, Kitchener locations):The NP is a full member of the Interprofessional team of WRNPLC and of the collaborative Cambridge/Kitchener Rapid Access Addiction Medicine Clinics (RAAM) The NP provides comprehensive primary care services (3-4 days) including the individual throughout their life span, the family, and the community; with a strong emphasis on health promotion, patient education and disease prevention. In addition, the NP will provide specialty addictions care at the RAAM sites (1-2 days). The NP provides primary care functions and participates in the development, implementation, monitoring and evaluation of programs and services both at the WRNPLC home base and in the RAAMs.

REQUIREMENTS:

· Baccalaureate of Science in Nursing required, Master of Science in Nursing

· Current registration with the College of Nurses of Ontario as an RN(EC)

· Current Membership with NPAO

· Evidence of liability insurance

· Current Ontario Driver’s License

· Demonstrated experience in Community and/or Primary Care Setting

· Demonstrated experience in working with vulnerable populations including mental health and addictions

· Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction

· Superior organization, research, evaluation, time management, communication and interpersonal skills

· Proficiency in the use of computer hardware and software, particularly in Microsoft Office 365

· Prior experience and knowledge of electronic medical record (Practice Solutions experience an asset)

· Desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops and conferences

· Multi-tasking skills related to the delivery of efficient primary care including the ability to share information and teach while treating a patient

· Excellent verbal and written communication skills

· Exhibit an ability to be open and non-judgmental

Addictions specific competencies (training will be provided and supported by NPLC and House of Friendship):

· Demonstrates an understanding of the neurobiology of addiction

· Assess and manages patients with addictions who have co-existing health conditions including but not limited to mental health conditions, blood borne diseases and infections

· Advanced health assessment to identify clients’ emotional, cognitive and behavioural states, as well as level of anxiety, crisis states, indices of aggression, self-harm, suicide, risk to others, competency to care for self, and signs of substance abuse, addiction, and withdrawal

· Utilizes evidence‐based interventions, including withdrawal and other medication management, to meet the needs of a diverse range of clients with addiction problems

· Provides care to persons experiencing an addiction and / or mental health condition that is recovery oriented, trauma-informed, uses principles of harm reduction, and addresses social determinants of health.

· Engages clients in strengths-based care that promotes resilience

· Provides education to individuals and families about addiction and its management. Facilitates and engages in collaborative, inter- and intra-professional, and intersectoral practice when providing care for persons with an addiction(s).

· Understands stigma as a barrier and assists and supports patients and families

· Acts as a resource providing addiction‐related clinical advice for health and other professionals, community groups and clients and their families

· Advocates for persons experiencing an addiction.

· Identifies and seeks new knowledge, skills, and supports related to care of patients with addictions and/ or mental health conditions.

Compensation: $118,000-$125,843
