Nurse Practitioner-Led Clinic Recognized at Health Canada Task Force Summit in Ottawa, March 11, 2015
WWLHIN_2015_ 1410Waterloo Region Nurse Practitioner-Led Clinic (WRNPLC) was selected out of 36 submissions to showcase its Anticoagulation Management Clinic Interprofessional Team at a National Summit in Ottawa on February 18, 2015.

An outline of the Clinic’s Anticoagulation Management Clinic was submitted to Health Canada by staff Clinical Pharmacist Evgenia Oreshkin. This outline highlighted WRNPLC’s innovative, patient centered approach to providing effective, easy-to-access care for its patients who receive the blood thinner Warfarin. The Clinic was presented to a large audience highlighting the importance of open communication and teamwork in primary healthcare.

The Anticoagulation Management Team consists of a Clinical Pharmacist, Nurse Practitioners, and Registered Practical Nurses and provides a “one stop” solution for patients who require ongoing anticoagulation management. Traditionally, patients on Warfarin have request blood tests from a vein at a lab and wait 24 hours for the results. With this program, patients come directly to the clinic, receive only a finger prick and results are instant. The Pharmacist can make any dose adjustments necessary right away and the patient has access to the entire team if they need further care.

Recognition at the National Summit highlights the mission of WRNPLC which is to be “an innovative provider of person centered, quality driven, integrated and accessible primary care”.

Conestoga College Leadership Award In August, Clinic Director Paula Carere accepted the “Leadership Award” on behalf of WRNPLC. This award recognizes the collaborative work we have done and continue to do with students from both the Nursing and Health Informatics Programs at Conestoga College.

“Jerry Gerow Nurse Practitioner Leadership Award” Congratulations to our very own Paula Carere who brought home this award presented by the Nurse Practitioners’ Association of Ontario.

Paula’s passion and determination are second only to her compassion for our patients. Her leadership guides us all here at the clinic and inspires us to be the best that we can be. There is no one more deserving of this award, congratulations Paula!!!
